Aaaaand ... that sucked.
Went through all of the useful registry keys ( wiki.winehq.org/UsefulRegistryKeys ) and got nothing that affected performance by any large percentage. At 1024x768 I'm getting about 15 frames per second; jump up to 1280x1024 and it drops to about 4.
Oddly enough, though, changing the DirectDrawRenderer to gdi from opengl didn't have any effect on performance. Unless I miss my mark, that suggests that the video performance is not the bottleneck. I wonder if the sound issues have something to do with it.
