double-mouse-pointer under steam is something that I've just been putting up with; by itself it's a little annoying but doesn't make the game unplayable for me. (See also bugs.winehq.org/show_bug.cgi?id=27779 )
The limited rotation is a thing that I see *sometimes*. There are a few things I try:
1) load a menu screen (like inventory), move the mouse around in there, and then resume the game and see if that changes it
2) wait around a bit, sometimes the game is unresponsive at first but then gets better (maybe it's loading maps or textures or something, who knows)
3) if all else fails, quit DX:HR (but _not_ wine) and re-launch it from Steam, sometimes it works better on the second time.
I think the patched wine 1.3.27 as you have worked okay for me, but you might also update to 1.3.28 (raw2.patch still required).
