What graphics card, what driver?
Tried disabling GLSL?
Running Guildwars in a virtual desktop?
Tried rebuilding your .dat file? (I had some problems with an old .dat that got a bit corrupted, delete your current dat - or move it elsewhere and run guildwars with the -update switch, eg
$ wine gw.exe -update
warning, the file is 3.7gig ish and will take some time to download.)
