This was with the Complete Pack version of the game, the version that installs the Deus Vult edition directly. As I installed over an existing uninstalled version, I did have one duplicate directory (CoA and COA) but it probably wouldn't have been a problem with a fresh install. (admin: changed to reflect experience)

How to install game fully patched
You have to change capitalization on some folders when installing an upgrade.
There are problems with capitalization on directories and files such as "gfx/CoA", /map/ADJACENT.TBL and "gfx/interface" when installing a patch or upgrading from normal Crusader KIngs to Deus Vult. This might leave you with for example two folders named "coa" and "CoA", rendering the game unplayable. The fix is relatively easy, but a bit tedious:
Unfortunately it's not always the same files, as different packages and patches use different names. However Deus Vult will choke on missing files, so if you've missed some you ought to be told upon starting the game.
