Windows EXE version, completely, as long as you install "winetricks vcrun2005" and an appropriate MP3 ACM; see notes.

As long as proper steps are taken, nothing except the proper display of the SHIFT_JIS title in the title bar (but that usually doesn't work in English copies of Windows anyway.)

The game MUST be run with the current working directory in the game directory. It uses relative paths to its data files, so if you run outside of the game directory it will fail to load graphics/sounds resulting in poor display / no sound.
The game requires MSVCP80.dll/MSVCR80.dll so you must install "vcrun2005" via winetricks. This will get the game to run, but it will have no sound. The reason is that Wine does not ship with an MP3 ACM module. Syobon quietly writes errors to a "Log.txt" file; messages regarding failure to load MP3 due to being unable to convert them to PCM (in Japanese) is because of the lack of (or improperly installed) MP3 ACM.
Checklist:
After installation of NT 4.0 NetShow Tools package manually add the following line to your ~/.wine/drive_c/windows/system.ini "[drivers32]" section:
msacm.l3codeca=l3codeca.acm
This should get sound to work. After that the game works flawlessly from start to end.
