What works
Help text, keyboard controls, flight. Graphics options must be set first by running cfs3config.exe followed by cfs3.exe.
What does not
First run always crashes. Second run always warns about previous crash but game runs fine.
Workarounds
Create a 32-bit wine prefix:
winetricks -q msxml3 verdana ie8
Some people suggest corefonts and msxml6 but I don't think they're needed. wine gecko doesn't work properly for in-game help text "F2" key. ie8 does work.
