Set [Game Options] Disable Movies=1 (in swkotor.ini or by running swconfig.exe) to avoid the cutscene crash. This may be necessary to start the application, as it plays intro videos. also had to make sure that "Enable Hardware Mouse" was unchecked (or set to 0) or my cursor was invisible. The cutscene movies, for the curious, can be played out-of-game with BinkPlayer, although it's nontrivial to figure out which files should be viewed at what point in the game.

Unfortunately after start the game I have an crash.
In console there is following error:
err:winediag:xrandr12_init_modes Broken NVIDIA RandR detected, falling back to RandR 1.0. Please consider using the Nouveau driver instead.
wine: Unhandled page fault on read access to 0x00000004 at address 0x5df5a0 (thread 00be), starting debugger...
Do you know how to fix that without changing video driver?

I can hear the main menu (by moving the mouse around) and I can hear the intro video. But the screen stays the same. Any suggestions? I tried both wine+nine and wine without nine, no change.

Try running in virtual desktop if that helps.
Try disabling steam in-game overlay.
Are you using a widescreen patch for swkotor?

I am using the widescreen patched exe (the other one does not work at all)
I figured out the problem: When I disable AA it works flawlessly.

1. create new prefix for kotor
$ winetricks prefix=$HOME/.local/share/wineprefixes/kotor winecfg
1a. let winecfg do its thing
2. Install Steam
$ WINEPREFIX=$HOME/.local/share/wineprefixes/kotor wine msiexec -i $HOME/.cache/winetricks/steam/SteamInstall.msi
3. Run Steam properly
$ cd $HOME/.local/share/wineprefixes/kotor/drive_c/Program\ Files\ \(x86\)/Steam/ && WINEPREFIX=$HOME/.local/share/wineprefixes/kotor wine ./Steam.exe -no-dwrite
4. Install Kotor (wait 20minutes)
5. Run it from steam (click play now).... game doesn't appear (it's running as visible with `ps -fe | grep kotor`)
Installed wine from ubuntu ppa.
$ dpkg -l wine | grep wine

I have PulseAudio running and is experimenting with suspending it, not running it, etc., but so far no go. Any and all hints and suggestions would be appreciated.

In the title screen and in game, the game was only in grey, apparently lacking textures. I read that it was a problem due to the lack of S3TC support, and that it was needed to disable it, or install a package that include it.
Disabling using driconf breaks other games, so I reverted that change.
The package pointed out was one from the deb-multimedia repository but there is also a free implementation on Debian official repository, the package libtxc-dxtn-s2tc0.
As my wine is from i386 (multiarch), I installed it with:
sudo aptitude install libtxc-dxtn-s2tc0:i386
