Additional Comments

First off, Path of Exile version 2.4.2 breaks on Wine 1.9.21 and above with Direct3D errors. Run on 1.9.20 or below, tested on 1.9.16, 1.9.18 and 1.9.20.

On 1.9.20, this game runs smoothly at an almost stable 60fps in 1920x1200 in windowed fullscreen on the following hardware:

Intel i3 4170 @ 3.7GHz
8GB Ram
NVIDIA GeForce GTX 750Ti

RUNNING THE GAME:
The key to removing microstutters during gameplay is to mess with the garbage collector with the -gc flag, per https://www.pathofexile.com/forum/view-thread/8980/page/166
I run the game using the following, with great success.
WINEDEBUG=fixme-all wine PathOfExile.exe -gc 100

This causes one major problem: the game will continue to hog memory until graphics and sound errors occur, and eventually the game crashes.
However, since I can get smooth gameplay for 3-5 hours before this happens, I just restart the game every so often to avoid it.

Note that this removes many of the effect and sound based stutters and FPS drops, such as a large amount of enemies shattering at once, opening lockboxes etc.

HOW TO GET THERE:

Works fine, although it takes a bit long to allocate space.

HowTo / Notes

Required fixes (as of 2014-08-07)
Set fullscreen
Edit the production_Config.ini file located in ${Documents}/My Games/Path of Exile/. Change fullscreen to true. This prevents the "Failed Resetting Direct3D device objects" crash.

Override openal32.dll:
In winecfg, set the openal32 library to native, builtin or native. This prevents errors involving sound files at launch.
Unconfirmed: An alternative is to launch Path of Exile with sound off with the -nosound flag.
Recommended fixes (as of 2014-08-07)
winetricks riched20.dll // for launcher
winetricks usp10.dll    // fix unicode missing symbols
