What does not

Graphics were a problem. Resolution would not go fullscreen on external display (only worked on laptop screen).
Game would crash and freeze the computer until I changed the renderer to openGL.

Workarounds

Game crashes and freezes on start up unless the renderer is changed to openGL before launching.

You can change the settings in UT2004.ini located at:

~/.wine/drive_c/GOG Games/Unreal Tournament 2004/System/UT2004.ini

Under [Engine.engine]

Change:
RenderDevice=D3DDrv.D3DRenderDevice

To:
RenderDevice=OpenGLDrv.OpenGLRenderDevice

Save and run the game. Immediately go into settings and turn all graphics settings to low to get best performance.
